8 Violation of Code: [46.981(h)] The peak hot water temperature at customers' restroom is 60 degrees F. Repeat Violation.
11 Violation of Code: [46.201] Two bags of 50 pounds flour are gnawed on a shelf in the bakery preparation area. The person in charge dscarded them at time of inspection. Corrected On-Site.
13 Violation of Code: [46.301] Mouse droppings are observed on two cutting boards in the main kitchen. The cutting boards are cleaned at time of inspection. Corrected On-Site.
14 Violation of Code: [46.712] Slicer and meat grinder has an accumulation of food residue on the food contact surfaces and is not being washed, rinsed and sanitized at least every 4 hours in the main kitchen. Repeat Violation.

A vacuum package machine is observed on a table in the deli department. The person in charge voluntarily discarded them. New Violation.
36 Violation of Code: [46.981(k)(l)] Mouse droppings are observed in food preparation area, storage area and retail area. Repeat Violation.
36 Violation of Code: [46.922(c)] Open rodenticide are observed on floor in the storage area. The person in charge discarded them. Repeat Violation.

Aluminum foils are used as shelf liners in the main food preparation area.
Rusty shelves are observed in all walk in coolers beside the one in meat department.
Bare concrete floor surface in the middle of meat walk in cooler is not smooth and easily cleanable.

45 Violation of Code: [46.673] Domestic microwave oven is observed in the deli department. Repeat Violation.
45 Violation of Code: [46.672] Cutting boards observed with deep scratches and scoring not allowing for effective cleaning and sanitizing in produce warpping area and main kitchen. Repeat Violation.
47 Violation of Code: [46.714] Mouse droppings are observed at the following locations:
-On shelves in the bakery aisle and pumpkin seed aisle in retail area, storage area and food preparation area (main kitchen)
-On dough mixing bowl holder in bakery preparation area.
-On two boxes of cat food and a bag of cat food is gnawed. A staff discarded the gnawed cat food.

49 Violation of Code: [46.825(b)-(e)] Leaky faucets are observed at handwash sinks in meat preparation room and in an employees' restroom next to produce storage area.

-water stained ceiling and wall surfaces in produce storage room and on a hallway next to meat preparation room.
-grease accumulation on wall and ceiling surfaces around baking oven.

-Cracked floor tiles in all food preparaiton area and retail area. Repeat Violation.
53 Violation of Code: [46.981(f)] Moist mop is kept in utility bucket next to the mop sink. Repeat Violation.
53 Violation of Code: [46.981(e)(m)] Brooms and dust pans observed in direct contact of the floor and not elevated or hung on a utility rack. Repeat Violation.
54 Violation of Code: [46.963] Employees' jackets and coats are hung on food storage racks in food storage area. The person in charge relocated them to the lockers' room. New Violation.
54 Violation of Code: [46.922(a)] A light bulb in the deli meat and cheese walk in cooler is not shielded from breakage.
Light tubes in the main food preparation area are not covered. Repeat Violation.
54 Violation of Code: [46.614] Excess grease build up is observed above an oven in bakery preparation area. The vent above baking oven is not sufficient to prevent grease build up. Repeat Violation.

This inspection has revealed that the establishment is not in satisfactory compliance. Corrective action is required to eliminate these violations. Compliance status will be assessed upon reinspection.
Due to imminent health hazards observed during this inspection, the establishment has been issued a Cease Operations Order and must discontinue food operations immediately. The establishment cannot operate for a minimum of 48 hours. The establishment cannot operate until the required fee is paid and the establishment is approved by the Department to resume operations. Please contact the Sanitarian at the phone number listed below for a re-inspection once the violations have been corrected and the fee has been paid.
You are required under Section 6-502 of the Philadelphia Health Code to correct the violations listed as noted above. Your failure to correct these violations may result in the revocation of your health license and other legal action. Reinstatement of a revoked health license will require payment of another license fee.
