8 Violation of Code: [46.981(h)] Old food was observed in the handwash sink.  New Violation.
8 Violation of Code: [46.941(a)-(e)] Soap and paper towels were not observed at the handwash sinks in the bar and in the basement food preparation area. Soap and paper towels were later replaced.  Corrected On-Site.  New Violation.
13 Violation of Code: [46.281] Large tub of tomato sauce was observed without a lid inside a refrigerator that was leaking non-potable condensation from the condenser. Tomato sauce was immediately discarded.  Repeat Violation.
14 Violation of Code: [46.711(a)] Dishes were observed soaking in old dish water in the food preparation sink. Dishes were removed and Sanitarian gave instructions to wash and sanitize the food prep sink.

Pink slime observed on the interior of the ice machine. Sanitarian gave instructions to destroy the ice and sanitize the ice slime affected areas.  New Violation.
20 Violation of Code: [46.385(a)(2)] Cheese, mayonnaise, and tomatoes (pico) were held at a range of 58-62 degrees F rather than 41 degrees F or below as required. Items were immediately discarded.  Repeat Violation.

36 Violation of Code: [46.981(k)(l)] Visible physical evidence of rodent / insect activity observed throughout the facility.

Mouse droppings were observed on floors in food preparation areas, near HVAC systems, and underneath the pizza oven.

Live American Cockroach observed on a glue trap with other dead American Cockroaches in the sump pump room with beer storage. Dead American cockroaches also observed in the employee restroom and next to HVAC equipment.

House flies observed flying in the basement food preparation area and in the dining area.

A large amount of fruit flies were observed throughout the facility. Fruit flies were observed at the bar, in the basement storage room, in the basement food preparation area, in the mop sink area, in the dry storage area for soda, and in the customer restroom area.  Repeat Violation.
37 Violation of Code: [46.321] Liquor room was observed with liquor stored in a room with an unclean urinal in the same room and was touching the shelving with liquor.  Repeat Violation.
45 Violation of Code: [46.671] A cold holding unit and a refrigerator were not in good repair.  New Violation.
46 Violation of Code: [46.613] No drain board or adequate storage shelving to allow for storage of soiled and / or clean items before and after cleaning at the warewash sink. Unclean dishes and utensils were observed in the food preparation sink and on top of and underneath a food preparation table with shelving.  New Violation.

Summary
Statements
This inspection has revealed that the establishment is not in satisfactory compliance. Corrective action is required to eliminate these violations. Compliance status will be assessed upon reinspection.

Due to imminent health hazards observed during this inspection, the establishment has been issued a Cease Operations Order and must discontinue food operations immediately. The establishment cannot operate for a minimum of 48 hours. The establishment cannot operate until the required fee is paid and the establishment is approved by the Department to resume operations. Please contact the Sanitarian at the phone number listed below for a re-inspection once the violations have been corrected and the fee has been paid.
You are required under Section 6-502 of the Philadelphia Health Code to correct the violations listed as noted above. Your failure to correct these violations may result in the revocation of your health license and other legal action. Reinstatement of a revoked health license will require payment of another license fee. An appeal to these orders may be made to the Board of License and Inspection Review, Municipal Services Building, 1401 JFK Boulevard Philadelphia, PA 19102-1617
