1 Violation of Code: [46.102] The Person in Charge did not demonstrate adequate knowledge of the Phladelphia Food Code. Facility is still washing utensils in the handsink without proper sanitization requirments. Sanitarian reminded the person in charge of the importance to use the three compartment sink or warewashing unit. Corrected On-Site.
8 Violation of Code: [46.981(h)] Soap was not available at the handwash sink in the back food preparation area. Sanitarian instructed the person to place the soap at the handsink. Corrected On-Site.
8 Violation of Code: [46.825(a)] The handwashing sink was observed being used to wash a knife. Repeat Violation.
14 Violation of Code: [46.711(a)] Black residue, observed on the interior of the ice machine bin at the beverage bar around the side. Repeat Violation.
14 Violation of Code: [46.711(a)] Dead cockroaches were observed in a can storage shelf, on a knife. Sanitarian instructed the person in charge to wash the knife. Person in charge washed the knife in the handsink with water. Sanitarian then instructed the person in charge to wash, rinse and sanitize the knife in the three compartment sink. Repeat Violation.
21 Violation of Code: [46.385(b)] Cooked rice and othe potentially hazardous ready to eat food, prepared in the food facility and held for more than 48 hours, located in the walk cooler, was not date marked. Sanitarian instructed person in charge to date mark the affected foods that weren't already date marked. New Violation.
26 Violation of Code: [46.1041] Clean pots and trays were observed stored directly on top of bottles of bleach. Sanitarian instructed the person in charge to rewash and properly store the items. New Violation.
36 Violation of Code: [46.922(e)(f)] The side entrance door of the facility is broken and has an opening, which is inviting to pests. Repeat Violation.
36 Violation of Code: [46.981(k)(l)] Visible physical evidence of insect activity observed in the kitchen and food storage area. House flies were observed in the kitchn. Nypmh German Cockroachs and adult were observed in the storage area in cracks of the wood shelving and in shelving units. Cockroach fecal matter was observed throughout the facility on walls. Repeat Violation.
36 Violation of Code: [46.922(c)] Fly tape was observed wrapped around light fixtures at the beverage bar area. Sanitarian instructed person in charge to remove the fly tape. New Violation.
41 Violation of Code: [46.302] Knives and / or cleavers stored between table edges which are not easily cleaned and sanitized. Repeat Violation.
45 Violation of Code: [46.671] Domestic reach in freezer was observed.
Hot water dispenser, used for tea, is leaking. (NOTE: leak is not at water intake line, it's actually coming from the equipment's dispenser column where you can see the water level inside).
Handle to reach in refrigerator is missing.

Summary
Statements
This inspection has revealed that the establishment is not in satisfactory compliance. Corrective action is required to eliminate these violations. Compliance status will be assessed upon reinspection.

Due to imminent health hazards observed during this inspection, the establishment has been issued a Cease Operations Order and must discontinue food operations immediately. The establishment cannot operate for a minimum of 48 hours. The establishment cannot operate until the required fee is paid and the establishment is approved by the Department to resume operations. Please contact the Sanitarian at the phone number listed below for a re-inspection once the violations have been corrected and the fee has been paid.
You are required under Section 6-502 of the Philadelphia Health Code to correct the violations listed as noted above. Your failure to correct these violations may result in the revocation of your health license and other legal action. Reinstatement of a revoked health license will require payment of another license fee. An appeal to these orders may be made to the Board of License and Inspection Review, Municipal Services Building, 1401 JFK Boulevard Philadelphia, PA 19102-1617
