1 Violation of Code: [46.101] The food facility operator shall be the Person in Charge or shall designate a Person in Charge and shall ensure that a Person in Charge is present at the food facility during all hours of operation.
There was no Food Safety Certified parent at the beginning of the inspection.
He arrived before the end of the inspection. Repeat Violation.
8 Violation of Code: [46.941(a)-(e)] - No handwash reminder sign was posted at the food prep area handsink.
It was posted. Repeat Violation.
11 Violation of Code: [46.201] -Two half gallon and ten quarts of milk were expired (10/13 - 10/15) in the walk in refrigerator on the customer self service shelves.
They were removed from the shelf to return to the distributor. Corrected On-Site.
20 Violation of Code: [46.385(a)(2)] 12 sticks of margarine were stored in the cold hold unit that was not operating at 76 degrees F ratyer than 45 degrees F or lower.
Some were labeled to keep refrigerated.
They were discarded. Corrected On-Site.
26 Violation of Code: [46.1021] Chemicals (air freshener and oil) were stored above or on the same shelf with food (candy), equipment, and/or single service articles in the cashier area.
The chemicals were moved away from the food products. Repeat Violation.
26 Violation of Code: [46.1026] Observed insecticides (open can of fly spray) not labeled by the manufacturer for food facility usage, in the rear storage area.
The can was removed from the store. Corrected On-Site.
36 Violation of Code: [46.922(e)(f)] The open front window is lacking an adequate screen. It does not protect against the entry of insects, rodents, and other animals. New Violation.
37 Violation of Code: [46.323] - Splashguard is lacking between the sanitizing compartment of the 3 basin sink and the Food Prep Sink. Repeat Violation.

54 Violation of Code: [46.614] -The exhaust hood is lacking filters.
-The gas oven/range is not 6 inches within the exhaust hood. New Violation.
59 Violation of Code: [18.02] Installation of an exhaust hood,fryer, grill, gas appliances & components and stove began without prior approval from the Health Department. Possibly a steam table.

Statements
This inspection has revealed that the establishment is not in satisfactory compliance. Corrective action is required to eliminate these violations. Compliance status will be assessed upon reinspection.

Alteration or construction began prior to plan submission and approval. You are required to submit a Plan Review Application with proper plans, drawn to scale, showing all design features, including all food service equipment specifications and the location of toilet rooms, plumbing fixtures, storage areas, and utilities must be submitted to the, Office of Food Protection. Due to imminent health hazards observed during this inspection, the establishment has been issued a Cease Operations Order and must discontinue food operations immediately. The establishment cannot operate for a minimum of 48 hours. The establishment cannot operate until the required fee is paid and the establishment is approved by the Department to resume operations. Please contact the Sanitarian at the phone number listed below for a re-inspection once the violations have been corrected and all outstanding fees have been paid.

You are required under Section 6-502 of the Philadelphia Health Code to correct the violations listed as noted above. Your failure to correct these violations may result in the revocation of your health license and other legal action.
